Sam’s Story

Meet Sam,

Sam lives in supported accommodation and receives dedicated in-home and community support from a consistent team.

 

When Sam joined our services, public outings weren’t possible.

 

Today, he attends weekly swimming sessions, goes bowling, visits animal farms, and confidently explores his community with the support of a consistent in-home team.

We started by listening and understanding. His care plan was designed around Sam’s unique needs, including his non-verbal communication style.

 

Our team worked alongside Sam’s family and Support Coordinator to create an individualised approach with the help of his Case Manager, Positive Behaviour Specialist, and Lead Support Worker.

From there, we introduced Sam to the public pool. With guidance, strategies, and consistent support, Sam began weekly swimming trips.

 

The results were immediate. Not only did his physical fitness improve, but his confidence soared. Outings that once seemed impossible, like bowling or visiting an animal farm, became regular, enjoyable experiences.

We regularly support participants living with autism, psychosocial disabilities, epilepsy, and behaviours of concern, using structured, individualised support strategies.
