Initial Assessment:
A client initially received 4 hours of weekly NDIS support. However, we recognised the need for a more extensive evaluation and proposed a 24/7 observation over a week to gain a full picture of the client’s needs.Service Provision and Reporting:
After obtaining the family’s agreement and signing a service agreement, we provided the necessary care and conducted an in-depth assessment over a 7 day period using a 24/7 model of care. This involved:Summarising the client’s patterns and daily needs.
